As an LPN, you will promote and restore patients’ health by collaborating with the Director of Nursing and with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families. You will assist others in the Care Family to develop, coordinate and perform the tasks of our Family Member’s Service Plan according to their preferences and abilities. Pay: $35 – $40 per hour

Responsibilities:

· Complete Licensed Practical Nurse (LPN) Assessments in accordance with state regulations, to include but not limited to full assessment of residents upon move in, after significant change of condition and annually. Documenting findings, resident status & interventions made as a result of this assessment

· Teaching of Med Aides/other staff as needed must be provided and documented

· Monitoring of resident condition

· Participation on the Service Planning Team and/or reviewing the service plans with date and signature in a timely manner

· Establish a compassionate environment by providing emotional, psychological, and spiritual support to residents, friends, and families.

· Promotes resident’s independence by establishing resident care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.

· Health care teaching and counseling for staff as needed

· Maintain safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.

· Maintain resident confidence and protect operations by keeping information confidential.

· Maintain nursing supply inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.

· Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies

· Supervise and monitor floor staff.

· Performs all other duties as assigned.

Qualifications:

We are looking for a Part Time or Full Time Rn. As a Registered Nurse, you will promote and restore patients’ health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. You will assist others in the Care Family to develop, coordinate and perform the tasks of our Family Member’s Service Plan according to their preferences and abilities. As the Life Enrichment Director, you are responsible for organizing activity programs at the community level for client facility residents and their families and help residents improve their daily living skills by promoting resident socialization, expanding their personal interests, increasing their physical activity and continuing their education. In this role we ask that you bring a sense of humor and professionalism, while being an example to others and helping with any questions other staff members may have.

Responsibilities:

• This position is responsible for coming up with new activities and fine-tuning current ones in accordance with regulations. You are responsible for planning events that get all residents involved to the level they may tolerate, including some of their families. Your plan will include daily and weekly activities and some off property outings. A sample list of activities includes arts, music, dancing, crafts, educational programs, and exercise classes.
o If any planning involves the kitchen staff and Chef, you will work with kitchen, activate and confirm their part. When planning activities, you will be in charge of putting everything on a calendar and include it with the facility’s newsletter, displaying a copy for residents.
o If it involves third part vendors, you will work with the vendor(s) to ensure they have everything they need for the activity and explain facility’s goal for hosting such activity.
• Assist with the creation of an activities budget, including managing current funds, getting additional funding and pay for events
• From time to time, this position will work with volunteers for various facility activities. This position will supervise volunteers during these activities. This position will also work in tandem with the Administrator to train, recruit, and schedule and keep track of all volunteers, including keeping track of their hours and performance based on the activities they conduct and how they interact with the residents
• Performs all other duties as assigned.

Qualifications:

Work Environment:
While performing this job, the Activities Director could be exposed to infectious waste, diseases, and body fluids which can contain AIDS & Hepatitis viruses. Chemical exposure is limited to cleaners, disinfectants, and personal care items.

As the Medication Technician you are responsible for ensuring that our family members who live in our Community receive their medications and health related items in a safe, secure, and timely manner. You will assist others in the Care Family to develop, coordinate and perform the tasks of our Family member’s service plan according to their preferences and abilities. Responsibilities:

  • Be an example of the Whitewood Gardens Philosophy, the Care Family, while acting in accordance with the regulations of the state.
  • Be proactive and ensure that you receive updates regarding the previous shift from the Med Aide & Caregiver.
  • Maintain Family Member records in our EMR, including but not limited to, obtaining current physician orders, clarifying orders, and alert and monthly/daily charting.
  • Orders, stores, and assists with medications according to facility policy and procedure and current physician orders.
  • Read/Report in our Communication Log and EMR for any changes that have occurred for any of our family members throughout the day.
  • Assist our family members with activities of daily living, environmental orientation, vital signs, first aid wound care, and communication with outside health care professionals.
  • Share your knowledge of infection control and universal precautions.
  • Communicate the care given in our EMR, adhering to facility policies and procedures.
  • Assist in maintaining a clean, comfortable, and safe environment for everyone in the Care Family. Ensure that the front door is always secure, if needed.
  • Communicate deficiencies observed in our family member’s apartments and facility to administrator. Complete Incident Report forms as needed for any unusual occurrences.
  • Observe and communicate any changes in family member physical or mental status.
  • Be thoroughly familiar with all emergency procedures, following the policies and procedures in the fire and disaster plans.
  • Maintaining a clean and organized Medication Room.
  • Supervises and monitors floor staff.
  • Performs all other duties as assigned.

Report the needs and concerns to the supervisor on duty. In addition, you will have other duties, such as food service and laundry.   • Provides all family member care needs and comforts of daily living, including but not limited to: engaging in activities of interest, providing restorative activities assigned by nursing programs necessary to maintain ADLs, including but not limited to Bathing, Toileting, Dressing, Eating, Etc.
  • Exemplary customer service to residents, peers, families, managers, and visitors
  • Report any changes in physical or mental conditions in a timely manner.
  • Assist with assigned tasks, including but not limited to: Measuring and recording vitals; gathering weights, make beds, cleaning closets, organizing, and ensuring cleanliness of resident living area.
  • Documents family member information timely, accurately, and confidentially according to the procedures set forth in the EMR system.
  • Demonstrate proper use of equipment and report equipment needs or repairs.
  • Performs food service functions to include: Preparing family members for meals, serving food, assisting with eating, after meal care, passing snacks and fresh water/ice; recording food/fluid intake, noting changes of eating habits and appropriately reporting.
  • Performs all other duties as assigned.
